Personal Profiling for Investment and Foreign Currency Deposits
We realize that every customer has a different idea about investment objectives and individual attitudes toward risks. We also know that when customers consider investment choices, it is important in managing their assets that they first ascertain their investment objectives and the degree to which they are willing to accept risk. Accordingly, we use the personal profiling questionnaire at Citibank as a means of asking customers about their objectives and attitudes toward risk. This enables us to suggest products that appropriately match each customer based on individual customer risk scores which are derived from details provided by customers in the questionnaire. Please be aware terms and conditions for applications pertaining to respective products must be met when applying to conduct an actual transaction.
The questionnaire contains 16 to 18 questions and it takes approximately 10 to 20 minutes.

Please note that based on your personal profiling we may not be able to offer you products if product risk level is greater than your risk score.
Please note that purchase of Mutual Funds, Bonds and Insurance products requires the ability to read and understand Japanese language documents such as Prospectus etc.
Please answer the "Personal Profiling for Investment and Foreign Currency Deposits" questionnaire if you wish to apply for or use any of the following products or services. After you answer the questionnaire, you will be assigned a customer risk score. Your responses are valid for one year. However, you will be asked to make revisions each time your financial situation changes, even if it has been less than one year.
Please note when you are attempting to purchase products and if you don’t have a risk score or your customer risk score has expired, we will notify you need to acquire a risk score. In such a case, please take the personal profiling questionnaire.

Where to complete the "Personal Profiling for Investment and Foreign Currency Deposits" Questionnaire
The "Personal Profiling for Investment and Foreign Currency Deposits" can be submitted using your choice of the following four methods. If your investment objectives or financial situations changes, we recommend that you take the personal profiling again to determine your appropriate risk score.
Via the Internet
The questionnaire may be conducted via Citibank Online.
In principle, you are allowed to complete the personal profiling questionnaire once a day. However, when providing the questionnaire via Citibank Online you may re-enter questionnaire details one more time on the same day to correct errors. Please be aware that after you acquire a risk score via Citibank Online, you may not conduct another questionnaire on Citibank Online for thirty days from day after submission of the questionnaire.
Sign on to Citibank Online
Click the menu option "Personal Profiling" from the left hand navigation.
Click "Conduct Profiling".
Answer the questions that appear on the personal profiling questionnaire screen.
By Phone
You can choose to answer the questionnaire using CitiPhone Banking (0120-110-330 from within Japan, or 81-45-330-2880 if you are overseas (telephone charges applied)).
After receiving a risk score, you will be presented with products that match your specific investment needs.
At Citibank Branches
You may complete the questionnaire in person at one of our branches.
After receiving a risk score, you will be presented with products that match your specific investment needs.
You may request the "Personal Profiling for Investment and Foreign Currency Deposits" by calling Citiphone Banking (0120-110-330 from within Japan, or 81-45-330-2880 if you are overseas (telephone charges apply). We will then send you the necessary forms.
Please fill out the "Personal Profiling for Investment and Foreign Currency Deposits" when it arrives and return it to Citibank in the enclosed return envelope.
In a few days we will provide information by mail regarding your risk score.
At Citibank, customers are charged a monthly account maintenance fee of 2,100 Yen (tax included), which is deducted from their account on the first banking day of the following month. However, this fee is waived when any of the below criteria is met:
the customer is paying a housing loan (including Mortgage Power),

the average monthly loan balance for Advance Money or card loans in the previous month is 4 Yen or more,
.
the customer is a member of the Citi Platinum Card or Citi Gold Card issued by Citi Card Japan Inc. (Citi Platinum VISA Card, Northwest/Citi WorldPerks Platinum VISA Card, Citi Gold Card, Northwest/Citi WorldPerks VISA Gold Card, Citi Elite, and Citi Dollar Card) and designates a Citibank account to pay balances on these cards, or

a new account is opened for the month in which the account is opened and the two subsequent months.
Foreign currency deposits are not covered by any deposit insurance.
