e-Form / CARD / PIN / T-PIN APPLICATION AND CHANGE REQUEST

During your session, please do not use Browser Back Button and use "Back" or "Cancel" button inside Citibank Online.

e-Form is a service to make a request to manage your information and service in Citibank Online.

Below are the instructions for making a Card / PIN / T-PIN Application and Change Request using our e-Form Service.

Select e-Form for Card / PIN / T-PIN Application and Change Request

Click “Customer Service” from the main menu on the left-hand side of the screen and select “Card / PIN / T-PIN Application and Change Request” below “e-Form”.

  • If you have not registered your e-mail address for Alert Service
    Please register your e-mail address for Alert Service in order to start using our e-Form Service. Clicking on “Register E-mail Address” will take you to the registration screen.
*By registering for Alert Service, you will receive an e-mail notification for the below items:
・Request received notification
・Cancellation notification of requests with incomplete detail
・Request completed notification

Read Notes

Please read “Notes”, “Terms and Conditions”, and click “Next”.

Clicking “Cancel” will end the session, and return you to the e-Form menu.

Input Request Details

Please read the below instructions carefully and enter request details. After entering the required information, please click “Next”.

Clicking “Cancel” will end the session, and return you to the e-Form menu.

Clicking “Back” will return you to the “Notes” page.

INSTRUCTIONS:

  • Please check all applicable item(s) and fill in the information.
<Input page 1>
(1) Reissue of Card

(1)
To reissue your card, please check the checkbox labeled “Card Reissue” and click “Next”.
(2)
Please then enter information details in entry form page 2.
*JPY1,050 per card will be charged for reissue.

(2) PIN / T-PIN Change

(1)
To change PIN and/or T-PIN, please check the checkbox labeled “PIN / T-PIN Change”.
(2)
Please check the checkbox labeled “PIN Change” and/or the checkbox labeled “T-PIN Change” to specify your request.
(3)
For PIN Change, please select an account to be processed from the dropdown list.
(4)
After entering all the necessary information, please click “Next” and proceed to the Review page.
<Input page 2> - Card Reissue only
(1) Account to be processed: Please select the account for which your card will be re-issued from the drop-down menu.
(2) Please place a check in all appropriate boxes to select which card is to be requested for reissue.: This application is only for the reissue of a card. To obtain a new card, please visit our branch office or make a request by mail.
Account Holder’s Card: Please check the box to request the reissue of Account Holder’s Card.
Supplementary Card 1: Please check the box to request the reissue of ONE supplementary card.
Supplementary Card 2: Please check the box to request the reissue of TWO supplementary cards.
(3) Card Type: Please select the card type for reissue from Magnetic Stripe Card and IC Card. An IC Card can only be issued for a Yen Savings Account or Yen Savings e-Savings Account.
(4) Reason for Reissue: Please select Reason for Reissue from the drop-down menu.
(5) Supplementary Card Holder’s Name: Please enter Supplementary Card Holder’s Name. For alphabet name, please enter the name in the following way: First Name, Middle Name, and Last Name.
(6) Supplementary Card Holder’s Furigana Name: Please enter Supplementary Card Holder’s Furigana Name. For alphabet name, this information is not mandatory.
(7) Account to be charged (JPY 1,050 per a card): Please select the account from which the replacement card fee will be debited. If the balance is insufficient, we will not be able to re-issue the card as requested.

Review Request Details and Submit the Request

Please review the information you have entered in the form. You can correct the information in the form by clicking “Back” if required.

After confirming that all the information has been entered correctly, please click “Submit”. Your request will be submitted successfully, and you will receive an e-mail notification to the e-mail account you have registered for our Alert Service.

Clicking “Cancel” will end the session, and return you to the e-Form menu.

Confirm Request Details

Please confirm your form entry details and reference number.

Clicking “View Record” will display your request details in printable format. Please click the “Print” button at the top right to print out the form entry for your reference.

Clicking “Back to e-Form Top” will return you to the e-Form Menu page.

Clicking “Back to Citibank Online Top” will take you to the Citibank Online Top page.

Process Flow after Request Submission

Request via Citibank Online will be processed within 3 – 5 business days. Requests submitted after 15:00 (JST) on business days and/or holidays will be processed from the next business day.

E-mail notification will be sent to your e-mail address registered for our Alert Service when your request has been processed successfully.

In case we cannot proceed with your request for an incomplete form or other reasons, the request will be cancelled and you will receive an e-mail notification for the cancellation in your e-mail account registered for our Alert Service. In such case, we will send detailed reasons for not accepting your request via “Citibank Online Message” in your Citibank Online account. Please check the message and make a new request with the necessary changes again.
